Reading The Report
How to Access Web Portal, Quality Reports, On-Line Survey and Honorarium Payment
| Step One |
Log on to website: www.p2quality.com |
| Step Two |
Click on the “register” button and a new web page will appear. |
| Step Three |
Enter your registration certificate number (provided at the bottom of page two). Click “Retrieve Personal Information” button. |
| Step Four |
Review demographic data. • If this data represents you and your practice, fill in any blank field and correct any wrong information (neither the DEA nor NPI number can be edited). Click “Update and Register” button. or • If this data does not represent your practice, click “This is Not My Information” button. A project manager will review your data and get back to you within four (4) business days. |

Access your email account and the message sent from My Quality Counts! |
| Step Six |
In that email, you will receive a temporary password and the web site link to the log-in page. Click the link. |
| Step Seven |
Once at the log-in page, you will be asked to enter your temporary password provided in the email sent to you. At that point, you will be able to create your own password and then click “submit” to be taken into the quality measures report. |
| Step Eight |
You now have the opportunity to review your quality measures report based on the data aggregated from the three participating health care plans. |
| Step Nine |
If you would like to view the affiliated patients associated with any and all your measures, you will need to print, sign and fax back a Business Associate Agreement (BAA) to the project manager. To access the BAA, click on the “View Affiliated Patients” button which will prompt a pop-up window that will allow you to print the BAA and fax it back to (716) 218-1738. It will take approximately 24 hours from the faxed receipt of the BAA to allow you to view the patient-level data. |
| Step Ten |
You are invited to complete an on-line survey. The survey is available by clicking the "Survey" link that is posted on the quality measures page. The survey will prompt you through the process, and the "submit" button on the final screen will send your survey results to our project manager for compilation. |
